Mid-Certs certifications are still being done over the phone. Please upload all required documents before or at the time of your appointment with the link sent to your phone, if documents are not entered before or at the time of your appointment your certification will not be completed and benefits cannot be loaded on your eWIC Card. Therefore, MUST HAVE ALL DOCUMENTS to receive your WIC benefits or infant formula. Required documents are: ID for person being certified, income information, Medicaid information and resident information. The link sent to your phone will only be available for upload for 24 hours, therefore, upload your documents as soon as you get link.
If you are pregnant, or would like to get back on ACL WIC Program, would like to transfer to our ACL WIC Program, or need to get re-certified, please call to
make an appointment at
(505) 552-6067/(505) 552-6068 so an appointment will be schedule for you, your child(ren) or infant.
Please bring the following documents: Proof of Income, Proof of Residency, Proof of Identity, Proof of Medicaid, and Proof of Pregnancy. Also must bring these required documents each time you, your child(ren) or Infant is being re-certified. Only a parent or legal guardian can bring child/children or infant for certification and re-certification.
Once paperwork is filled out our Nutritionist/CPA will then certify you, or your child(ren) or infant. The WIC Tech will get your child(ren), yourself or infants weight and hemoglobin. The interview process should take
15 to 20 minutes, unless you have more than one child being certified.
After certification is complete your eWIC card will be loaded with your benefits for the month or months. If you are new to the ACL WIC Program, the allowable foods to purchase will be explained to you. Also an ID Card will be issued to you for benefits redemption purposes. You or your proxy Must bring your ID Card to our office reloading of your eWIC Card
Remember, for those infants that receive special formula, we need a prescriptions every three months. If your not sure if you need an updated prescription, please contact the WIC Office.
The ACL WIC program provides the above services to the women,
infant and children in the communities of Laguna, Acoma, and Canoncito. We also are able to provide WIC services to all ethnic groups in the surrounding communities of San Fidel, Cubero, Seboyeta, Highland Meadows, Albuquerque and Grants, Milan and San Rafael.
